Requirements

In order to begin work or deliver materials on any of our job sites, trade partners must submit up to date copies of the following documents:

1. Commercial General Liability Certificate – $2,000,000 aggregate coverage, $1,000,000 per occurrence.
2. Please submit a copy of CG2010 and CG2037 or equivalent with Craig Construction listed as additional insured.
3. Workers Compensation Certificate – Per statutory requirements and limits.
4. Automotive Insurance – $1,000,000 coverage on all vehicles that will be on site or driven to and from site.
5. W9
6. Actual copy of Utah State Contractors License (if required)
7. Actual copy of Utah State Tax License (if required)
8. Professional Liability Insurance Certificate (architects and engineers)
9. Actual copy of city/county business license.

You will be asked to send updated copies if/when your insurance expires. Please cooperate in this matter by sending the information in a timely manner.
